Insurance and Out-of-Pocket Rates

If you choose to utilize your health insurance benefits, I am in-network with Humana, United Healthcare, Aetna, and Caresource Marketplace. I can also provide you with receipts so that you can seek reimbursement through other insurance carriers by using your “out of network” benefits if you are with a plan not listed here.  When utilizing insurance benefits, you simply pay your co-payment and/or deductible.  I will take care of billing your insurance company.

If you have decided that you prefer to have full control over your treatment and your mental health record and wish to pay out of pocket, my rates are $135 for the initial evaluation and $125 for each subsequent session.  Each session is 55-60 minutes.

I accept cash, checks,  all major credit cards, and health savings accounts as forms of payment.
